When you change your name (or your child’s name) in Victoria, we send you a certificate with the new name on it.
This page is for people who need to order another certificate later – for example if you lost it and need a replacement.
We can only issue a certificate if you have already registered a change of name in Victoria.
What certificate will I get?
If you were born in Victoria, you’ll get a new birth certificate.
If you were born overseas but changed your name in Victoria, you’ll get a change of name certificate.
If you changed your name interstate, contact the Registry in that state or territory.
